Special note for item 8.: Item 8. is to be completed only by property owners who either wish to completely stop getting all landscape care services that are provided by FHVPOA or who wish to completely stop having all chemical treatments that are provided by FHVPOA applied on their property. It should be clearly understood that choosing either of the STOP options does not alter the property owner's legal obligation to pay FHVPOA the full amount of assigned monthly assessments.
